Referral Guide
Important Notice: No Walk-Ins or Emergency Services
SEVO-Med is a specialty referral practice. We are not an emergency hospital and do not accept walk-in patients. All appointments must be scheduled in advance through the referral process.
Referral Process
1. Submit a Referral
Choose one of the following methods:
Online: Complete and submit our referral form via our referral page. Attach medical records, diagnostic results, and relevant treatment notes. Records can also be emailed to [email protected].
Portal: Use our rVetlink portal for secure, 24/7 patient record access and eReferral submissions.
Phone: Call Orange Park (904) 278-3870 or Jacksonville (904) 567-7519 to provide case details and coordinate patient scheduling.
For Oncology referrals, a definitive diagnosis is preferred. However, if a diagnosis is unable to be determined prior to consultation and a mass is present within the thoracic cavity, abdomen, or nasal cavity, our specialists have the ability to perform the necessary diagnostics to establish a diagnosis before proceeding with treatment. If uncertain, request a specialist consultation before referring.
2. Appointment Coordination
For online submitted referrals our team will contact the pet owner within 24 business hours of receiving the referral.
If a case is urgent but not an emergency, call us for specialist communication. However, immediate availability
is not guaranteed.
Incomplete medical records may delay scheduling until all required documents are received.
3. Pre-Appointment Instructions
Clients will receive confirmation and preparation guidelines before their appointment.
Non-diabetic patients must fast before consultation. Please inform pet owners accordingly

Post-Consultation Communication
A detailed report will be sent within 24-48 business hours and will also be available on the rVetlink portal.
Diagnostic results may take several days; reports may be delayed accordingly.
Our specialists remain available for follow-up discussions to ensure continuity of care.

Emergency & Urgent Referrals
If a case is rapidly declining, call us to request a specialist consultation.
We will attempt to accommodate urgent cases based on availability but cannot guarantee emergency appointments0.
Our specialists and nursing staff can provide guidance if emergency consultation is unavailable.
